DEPARTMENT: Care Navigator Team
REPORTS TO: Care Navigator Manager
WORK WEEK: 40 hours/week, with
occasional coverage on nights and weekends on an as-needed basis.
ROLE DESCRIPTION:
In our model, Care Navigators, much like Community Health Care Workers, provide
structured advocacy, social support, and navigation to patients in order to help them reach their health and
life goals. Care Navigators are trained lay people who share life circumstances (race, socioeconomic status,
language, etc.) with their patients. They are “natural helpers” within the community. We are looking for devoted
community members who are passionate about health advocacy and patient engagement.
DUTIES AND RESPONSIBILITIES:
Magnet Care Navigators work exclusively via phone to assess Members’ needs, complete
health risk assessments, and connect Members to the MyOwnDoctor platform for virtual care and to any educational
resources and social services they need.

- Follow-up until
registration and enrollment processes are complete
- Engage in
relationship building and collaborative goal setting
- Ensure completion
of baseline health information and social needs data
- Use tools such as
motivational interviewing to improve enrollment, engagement and retention rates
- Act as a health advocate and help members reach health
goals

- Track progress with
goals using initial screening and ongoing assessment of health behaviors
- Assist in
self-management and identification of family and social support systems
- Enhance care
quality by aiding communication between provider and patient to clarify cultural
practices
- Educate Members
about how to use the health care and social service systems
- Help maintain an
up-to-date directory of community resources

social and health care support by referring, conducting “warm handoffs,” and following
up to close referral loops
- Refer and link to
preventive services through health screenings and healthcare information
- Professional Documentation and Collaboration

QUALIFICATIONS:
- High School Diploma / GED (or
higher)
- Must have resided within the local
community (Chicago or surrounding areas) for 2+ years and demonstrate:
- Knowledge of culture and
values of community
- Familiarity with the
resources available in the community
- Excited about how technology can
support your work and help drive the ongoing evaluation toward new and better care
- Comfort with using and learning new
technology
- Must be available to share coverage
with the entire team for our operating hours of 8am - 8pm weekdays
- Preferred:
- Community Health Worker
Certification/Accreditation or equivalent degree
- At least 1 year of
experience working as a Community Health Worker, Patient Navigator or similar patient-facing
position. (Volunteer and internship experience will also be favorably considered.)
- Experience working with
Medicaid populations
- Experience conducting
Community Health work virtually
- Willingness to commit to at
least a full year of employment
PLACE OF EMPLOYMENT:
The position is offered temporarily as a remote work arrangement, and Company
will transition into a full-time in-person or hybrid in-person and remote work arrangement when it is deemed
appropriate. Company will communicate its expectation regarding work to be performed in-person. Company may
change the work location from time to time at its discretion or terminate the remote work arrangement at its
discretion.
Proof of a COVID vaccination is required. For any in-person work or training,
proof of a negative test result within 24 hours is required.
